In New Jersey, partners in the Transforming Pediatrics for Early Childhood (TPEC) program are using the HealthySteps model to strengthen pediatric care for young children and their families. Participating pediatric practices are increasing developmental, behavioral, and social needs screenings, as well as screenings for parental and caregiver depression, to identify needs earlier and connect families with appropriate services. To strengthen implementation, practice teams participate in ongoing quality improvement coaching, using data to track progress, address challenges, and improve screening and referral processes.
In this video, New Jersey TPEC team members also share that by integrating family resource specialists into pediatric care teams, partners are providing personalized support to help families navigate early intervention services and access community resources. Families shape improvements to care through patient and family advisory councils and input on practice resources and environments.
In New Jersey, implementing HealthySteps alongside continuous quality improvement and family engagement strategies is helping create more responsive, coordinated pediatric care that supports the well-being of children and their caregivers.
